Idea Exchange: It's time to start making sure you document the time
A reminder that timing matters when it comes to patient records.

It should be standard practice that when you make an entry in a patient file-no matter what the entry is about-you note the time. That helps team members know when something occurred with the patient. Also, for discharging purposes, it is important to document in the patient file who is allowed to pick up the patient, and when you do discharge the patient, who actually did pick up the patient and at what time.
