 feline medicine page is a comprehensive resource for clinical news and insights on the latest in veterinary feline medicine. This page consists of videos, interviews, articles, podcasts, and research on the advancements and developments of therapies for feline medicine, and more.

In June 2024, compounded GS-441524 became available by veterinary prescription for veterinary offices as well as individual patients permitted in the United States, giving veterinarians in the United States access to a feline infectious peritonitis (FIP) oral treatment. The compounded oral medication has come from an exclusive partnership between Stokes Pharmacy and Bova Group and is the first legal FIP treatment in the US.

On this week's episode of The Vet Blast Podcast presented by dvm360, our host Adam Christman, DVM, MBA, welcomed guest Christopher G. Byers, DVM, DACVECC, DACVIM (SAIM), CVJ, to discuss recent advancements in FIP treatment, emphasizing the importance of accurate diagnosis, and more.

: Historically, treatment of FIP has included medications like prednisolone, interferon and polyprenyl immunostimulant, and as the listeners know, these treatments have been largely unsuccessful and often resulted in euthanasia. Our colleagues in the United Kingdom and Australia have had legal access to something called GS 5734, more commonly known as remdesivir. This is a prodrug of GS 441524, with an extra chemical side chain that is supposed to improve cellular penetration. They're really lucky. They have access to both injectable and oral forms, and there are established protocols in these geographic locales for each route of administration. 

In the USA, we currently have no FDA approved treatments for treatment of FIP. However, Stokes Pharmacy, a PCAB accredited compounding pharmacy, has recently established an exclusive partnership with the Bova Group to offer a US made oral treatment for FIP so American veterinarians can now manage their FIP patients with Stokes Pharmacy's compounded preparation that is supported by Bova's, unique drug formula that's been utilized in clinical research studies across the globe.

Basically with cats, a lot of times, less is more you know? So you do not want to push it with cats. I would recommend anyone starting as a [veterinarian], definitely become Fear Free certified. I think the information you gain from becoming Fear Free certified is just so important and you can kind of think about things in a different way so try to incorporate Fear Free techniques with your kitties. Then taking that a step further, you can get this cat-friendly certification through [American Association of Feline Practitioners]. Doing [this certification] was really eye-opening for me as well it kind of brought all these other issues to light.

GFI #256 and compounding medication is the topic of this episode of The Vet Blast Podcast. Marcy Bliss, CEO of Wedgewood Pharmacy, joins Adam Christman, DVM, MBA, to explain the ins and outs of this new FDA guidance.

: Over many years, FDA has been asserting that it should regulate compounding from bulk drug substances in animal health. So to facilitate that, they've recently released a final guidance to kind of frame up their current thinking, and this guidance is called the GFI #256....Now, this is not a law or regulation. It's not a statute, but the agency has announced its intention to enforce primarily to pharmacy enforcement...For me, I think the broad intent is to ensure that if a veterinarian can use commercial products that they do so, and that compounding is used for its intended purpose purpose, which is really to provide customized medication when the veterinarian determines that an FDA-approved product is not the best treatment for patients. So, either it's a compliance issue, like you said, or the drug isn't available in the marketplace, like potassium bromide, or the drug needs to be changed...Maybe it's available as a tablet and it needs to be an injectable or damaged drug.

So the FDA, I think, really just wants to ensure the appropriate use of compounded medication. Of course, we believe veterinarians are already using it appropriately, but the FDA wants to put this guidance in place, I think, to put some guardrails and a framework around that.

What you need to know about GFI #256 and the changing landscape of compounding medicine

Speaking on The Vet Blast Podcast, Steve Dale, CABC, started with client misconceptions about pet allergies. Pet parents might expect cats (and dogs) to sneeze because that's what people are used to with human allergies. He also mentioned that clients usually believe their cat's allergies are food-related. He said, "When people go online, they automatically say 'food...' It's not always the food. In fact I would argue it's typically not the food." He then warned that without proper education, people will waste money going down the wrong path. 

Flipping the topic on its head, Dale spoke about a breakthrough product that actually helps people who are allergic to cats. "There's a food out, a cat food, that prevents people from being allergic to cats. What it does...it neutralizes the protein on the cat's saliva that people are allergic to...To me...it's mindblowing. To me, this is the best thing for cats since catnip."

Episode 78: A deep dive into cat allergies

Kash Kuruppu, DVM and chief veterinary officer at Basepaws, goes into detail about the company's oral microbiome testing and how it can help veterinary professionals detect dental disease earlier. 

"Currently we're able to look at the oral microbiome and detect tooth resorption risk, halitosis risk, as well as periodontal disease. So as vets, we already know that if we're seeing tooth resorption or any dental disease in really early stages, there's likely a genetic component."

Kuruppu added that early detection is just the first step. Basepaws plans to take the data it is accumulating and apply it to actionable reccomendations for clinicans and pet parents. 

"Now what we're actually thinking about is how do we affect change? How do we actually do something if we know the risk? It's one thing to know that you're predisposed...Great, but what do I do? And that's where veterinarians really want to know, 'Okay, what can I recommend? What kind of interventions should I be recommending and implementing?' And pet parents want to know this as well. So that's kind of the next step, not just for Basepaws, but the industry as a whole," said Kuruppu

Episode 77: Tools for predicting underlying and future dental disease

Genetic testing is used in everyday practice to identify risk factors for polycystic kidney disease, myelopathies, and some known coagulation disorders in cats. But now, more and more clinics are offering it as a part of their feline wellness plans, according to Kaushalya Kuruppu, DVM, chief veterinary officer of Basepaws.

In this latest episode of The Vet Blast Podcast, Kuruppu talks with Adam Christman, DVM, MBA, about the benefits of feline genetic testing. She also explains why and how offering this tool as a part of your patient’s wellness plan can improve their health and potentially extend their lifespan.

Genetic testing adds a lot of value for your client because it helps to initiate that often challenging discussion with cat owners about potential pet health issues. Some clinics offer genetic testing kits like Basepaws as either part of their wellness plan or as a separate add-on that clients can purchase in their retail section. “Our research team is [currently] developing a veterinary-specific test that we can do in the clinic and can get the report to share with your clients,” says Kuruppu.

It’s also important to note that just because a patient has or doesn’t have a health marker doesn’t mean that they will automatically develop the disease or that they’re out of the woods, warns Kuruppu. For example, “just because they are clear for kidney disease, doesn’t necessarily mean that they won’t get it based on their diet and other environmental habits,” explains Kuruppu.

Episode 36: How and why to include DNA sequencing in feline wellness plans

, according to veterinary dentist Jan Bellows, DVM, DAVDC, DABVP, FAVD. And half of the half that does have tooth resorption, will have it more than once, he explains. Although tooth resorption is less prevalent in canines, “a good 20% of the dogs that we’re seeing [in the clinic] will have tooth resorption,” says Bellows.

In this latest episode of The Vet Blast Podcast, Bellows offers insight on 2 classifications of tooth resorption and how to best manage this common condition.

You can classify tooth resorption by the anatomic extent of the lesion or by the radiographic appearance, Bellows tells Dr. Adam Christman. The anatomical approach includes the following 5 stages:

. Microscopic, it starts at the cementum and is hardly visible.

.There’s a visible bump with small amounts of tissue below it.

When it comes to tooth resorption, radiographic classification is arguably the most important and can be broken down into 3 different types.

. The has tooth resorption in it but the root has the same opacity as the adjacent teeth. You often have to remove affected teeth.

. There is tooth resorption, but the root is being resorbed with less opacity. The bone is replacing the root itself. If you wait too long, there won’t be any root left. “Since it’s exposed to the oral cavity, the tooth itself is painful,” explains Bellows. “So, you do a little flap, you take your drill, amputate the crown and a little bit of the root, and then you sow over the gingiva and you give the body it’s time to take away the whole root.”

 This is a combination of both type 1 and type 2. One root is type 1 and the other is type 2. “Since this is a combination of the first and second type, you can take out 1 root and perform a crown reduction on the other root that’s disappearing," says Bellows.

Episode 32: A look at 2 different tooth resorption classifications

As the host of two nationally syndicated radio shows and as an acclaimed pet journalist, animal behavioralist Steve Dale, CABC, shares some breakthroughs in feline research and offers a sneak peek at some of his upcoming Fetch talks that he will co-present with WSAVA One Health Committee Chair Michael Lappin, DVM, PhD, DACVIM (SAIM).

Addressing numerous trends in the feline world, Dale bookmarks the influx of cats in shelters, naming cat allergies as a key contributing factor.

“The no. 1 allergy on the planet isn’t hay fever, it’s not allergies to peanut butter… it’s cat allergies,” he tells Adam Christman, DVM, MBA. “If we can figure out a way that people aren’t allergic to cats, they don’t have that excuse to give up cats.”

, which he and Dr. Lappin will discuss in more detail at the upcoming virtual Fetch conference. This specially formulated diet is made with a protein sourced from eggs that contains an anti-Fel d 1 antibody. Most people who are allergic to cats, react to Fel d 1, a protein that is transferred to the cat’s hair and skin during grooming and eventually shed into the environment.

‘It’s the biggest scientific research news in the 30 years I’ve been doing this… It could save cats' lives,” Dale says.

He also talks with Dr. Christman about a new treatment for feline infectious peritonitis (FIP) and shares some other common misconceptions about cat behavior and ownership.
